Software Transfer of Ownership
Transfer an existing DEXIS Imaging Suite license and any modules to you as the new owner after a practice acquisition.

DEXIS software licenses are generally non-transferable.
This form is intended to be used in the limited circumstances, such as acquisition of a dental practice that licensed DEXIS software, at the time it was acquired.
By placing an order for software transfer of ownership, the new licensee represents that it has acquired from the former licensee the software refrerenced above and that it has the former licensee's permission to transfer the license to a new licensee.
DEXIS reservices the right to deny any requested transfer of the license for any reason or no reason.
Any transfer of license is subject to the terms and conditions of the transfer of license of DEXIS Imaging software.
A DEXIS Account Manager will contact the new software owner to discuss the extended warranty if one is attached to the software.
